What we do

Community Rehabilitation is a community-based therapy service for adults over the age of 18 who would struggle to access outpatient appointments or where home is the most appropriate place for their assessment or rehabilitation.

It is a community rehabilitation service including physiotherapists, occupational therapists and Assistant Practitioners working together with the therapy assistants.

We provide a rehabilitation service to allow our patients to achieve their goals and reach their optimum potential within the community.

Patients can access us at any time from their homes to support long term condition management. We also support hospital discharges and hospital admission.

What we don’t do:

Assessment for adaptations (stairlifts, level access showers etc) and specialist equipment.

Referrals for equipment only.

How to access the service

Referrals come from many sources. These include GPs, Consultants and the wider multi-disciplinary team such as District Nurses, Speech and Language Therapists and Integrated Neighbourhood Teams.

We accept patients with a GP from Blackburn with Darwen or East Lancashire locality. 

We accept referrals from the acute and community wards.

We also accept self-referrals.

The Single Point Of Access for patients (SPOA) can be contacted on 01282 805620.

We have locality bases to cover Blackburn with Darwen, Burnley, Hyndburn, Pendle, Ribble Valley and Rossendale who can all be accessed via the SPOA.

Most of our services are offered in patient’s home/ residence apart from certain clinics and classes but if these are offered to patients the location will be clearly stated on a letter.
