7) There is an Intra Ocular Pressure (IOP) referral refinement & repeat visual fields scheme, delivered by accredited practitioners within community optometry services. For patients with IOP > 31mmHg there is a direct urgent eye referral to Hospital Eye Services. For patients with pressures of 24-31 mmHg or where pressures which differ between the eyes by 6mmHg or more, a second repeat pressure is undertaken within four weeks. At the repeat reading, where the threshold of >24mmHg is noted, then referral to Hospital Eye Services is made.
For patients who are identified as having suspicious visual fields during a sight test will have visual fields repeated on a separate occasion by an accredited practitioner within community optometry services. If the second repeat tests show results within normal limits but patient is within at risk group / suspicious visual field then the patient remains within the community service for monitoring at appropriate intervals. Where visual field defect is confirmed, then the patient is referred to Hospital Eye Services.
Once referred to Hospital Eye Services, further tests are undertaken to confirm diagnosis. Patients are reviewed by doctors/optometrist/orthoptist, under the direction and supervision of the lead consultant (face to face / virtual clinics). Treatment is commenced as appropriate.
The pathway enables step down of ocular hypertension (OHT) / glaucoma suspect (GS) patients to accredited practitioners in the community. Patients who have hospital eye service confirmed diagnosis of ocular hypertension / glaucoma suspect and who do not require treatment will follow the OHT / GS monitoring pathway in the community (and can be transferred back to Hospital Eye Service if there is a change in clinical status i.e. target IOP surpassed and / or repeat visual field defect / disc status, or any other clinically reasoned concern).
8) Confirmation of diagnosis and monitoring is undertaken within Hospital Eye Services. It is supported by community optometry as outlined above.
