What we do
The Community Rehabilitation team works within the community to support patients that have deteriorated in their ability to manage at home because of a change in their health.
It is a community-based therapy service for adults over the age of 18 who would struggle to access outpatient appointments or where home is the most appropriate place for their assessment
The service provides a person-centered assessment, advice and rehabilitation programmes involving the patient based on what matters to them about remaining independent within their own home and their local community. This includes those who have lost confidence and ability with their activities of daily living due to ill health or living with a long-term condition and wish to remain at home requiring some rehabilitation and therapy interventions to regain their skills and achieve their goals and maximum potential.
The service’s main aim is to support patients with physical health problems, for example lung or heart conditions that may be affecting their mobility or ability to manage daily activities. This includes people who have fallen or are at risk of falling. Patients are visited in their own home and an assessment to discuss and check for environmental and other contributing falls risks, is completed. We will then discuss options for reducing the risks and managing falls.
Patients can access us at any time from their homes to support long term condition management. We also support hospital discharges and hospital admission.
The service consists of the following staff:
-
Occupational therapists
-
Physiotherapists
-
Assistant Practitioners and Therapy Assistants
Colleagues work together to support patients in the community and work closely with social services, GPs, community nursing teams, patients, relatives and carers.
What we don’t do:
Assessment of adaptations (stairlifts, level access showers etc) and specialist equipment.
Referrals for equipment only.
Please note the community rehab team is not an emergency or medical service – please ring the appropriate services if this is the case.
How to access the service
Referrals come from many sources. These include GPs, Consultants and the wider multi-disciplinary team such as District Nurses, Speech and Language Therapists and Integrated Neighborhood Teams.
We accept patients with a GP from Blackburn with Darwen or East Lancashire locality.
We accept referrals from the acute and community wards.
We also accept self-referrals.
