The Learning Disability Liaison Nurse Service works with people with learning disabilities and autism who are 16 years old or over.

Hospital Passport
Everyone with a learning disability is encouraged to use a Hospital Passport which helps inform hospital staff about the needs of the person with a learning disability and how to support them.
The passport is owned and controlled by the person themselves and should be brought with them when they attend hospital.
The passport also identifies the person who knows the patient best, such as a carer or a member of the hospital staff; they can provide valuable advice and support in understanding the person’s individual needs.

Call for Concern
Patients with learning disabilities and/or autism are at higher risk of becoming unwell as they often can't express how they are feeling as easily as others.
If you are worried that your loved one is deteriorating (rapidly becoming more unwell) and have tried to raise your concerns to the doctors and nurses on the ward but don't feel listened to - it may be appropriate to escalate via 'Call for Concern', also known as Martha's Rule. Call directly: 01254 735637.
Patients will also be asked daily about how they are feeling, this is called the Patient Wellness Questionnaire. If your loved one, who you advocate for, is feeling unwell, answering that they are feeling unwell can trigger a response from the doctors and nurses.
