Next week, between Monday, 21 June and Friday, 25 June, we will be working with our Health and care partners across Lancashire and South Cumbria to run a ‘Perfect Week’ initiative. The aim of this exercise is for us to focus as a whole health system on building from the learning of responding to the Coronavirus pandemic, test new ways of working and ensure people have the best possible experience of our health and care services. This includes primary care, hospital care, mental health, social care or the community care sector.

We are currently dealing with significant pressures from high volumes in A&E, dealing with a rising number of Covid positive patients and ramping up unscheduled care to try and reduce our waiting lists and see as many people as possible who need care and it is extremely challenging time. This initiative will help us to capture learning and build on our experiences to improve our services.

There are a number of identified areas of specific focus and these will be divided across the system to ensure we can complete a comprehensive ‘deep dive’. All learning will then be collated and shared across the Lancashire and South Cumbria Health and Care Partnership.

ELHT will have four main areas of focus:

Improving ED Processes

Improving Ward Processes (focus on timeliness of discharge/transfer)

Improving Frailty Identification and Management Processes

Launch of Enhanced Intensive Home Support Service Multidisciplinary team

Please do support this work by sharing the messages that we will be circulating throughout the week.
