A) 1088/1345 (81%) audited episodes required some sort of amendment to the coding. However, the majority of these changes were minor and the coding has consistently met Level 2/3 Data Security and Protection Toolkit (DSPT) scores. This includes both coder / non-coder (documentation) changes. Crude number of re-coded episodes is not a recognised measure of coding quality.

A) Yes we have a link to the national spine that updates GP information on referral. Other records are checked automatically against the spine and manually updated when issues are found.

A) We do not employ anyone to manage incorrectly delivered letters. Should any be received, they would be dealt with as part of staff members normal duties.
