A. This information is not able to be easily extracted from the Patient Administration System and would take longer than 18 hours to compile. Urinary catheters are only coded when certain conditions apply and not all are coded on the system. To check how many patients received an indwelling urinary catheter or were admitted with a catheter in situ would require checking all patient records for each of those years. In this instance we rely on the exemption given under Section 12 of the Act as this would take longer than 18 hours to complete. Accordingly, we are unable to provide the information for this question.

- Patients are given a catheter passport on insertion of a catheter providing education on catheter care and signs and symptoms of Infection. Staff complete a catheter care plan following HOUDINI guidelines.
b. Urinary Catheter selection, insertion and ongoing care? If so, please indicate what these are.
- We complete High Impact Interventions for catheter insertion and ongoing catheter care
c. What processes does the Trust have in place to ensure practical adherence to the guidelines in a) and b)? How does the Trust monitor these processes?
- The Trust divisions submit monthly High Intervention Audits for catheter insertion and ongoing catheter care, these are reported monthly on the DIPC report.
