1) The number of deaths of patients with a learning disability that occurred in the last three years (by years defined as 1/4/16 - 31/3/17, 1/4/17 - 31/3/18, 1/4/18 to present).

The above numbers represent patients with Learning Disabilities who are known / on the register and have died in our Trust. These cases are subsequently uploaded to the Datix system by our Learning Disability Team/Nurses and a Learning Disability Mortality Review will then be commenced.
Improved data capture of Learning Disability deaths can be seen by the increase year on year in the number of deaths that have been recorded and undergone a Mortality Review.
2) The number of deaths of patients with a learning disability reported as an incident on the National Reporting and Learning System (NLRS) in the last three years (defined as above)
1/4/16 - 31/3/17 – 0
1/4/17 - 31/3/18 – 0
1/4/18 – 31/3/19 – 0
The above data has been taken from the Datix system based on the patients identified in Question 1. None of these deaths of these patients were reportable as a death to the NRLS during the above specified dates.
3) The number of deaths of patients with a learning disability reported as an incident on the Strategic Executive Information System (STEIS) in the last three years (defined as above)

The above data has been taken from the Datix system based on the patients identified in Question 1. None of these deaths were required to be reported as a death within StEIS criteria during the above specified dates.
