Major Trauma Rehabilitation Co-ordinator

You recently have had a hospital admission and your injuries fall under the Major Trauma category.

When you were discharged from hospital you will have been referred for all your follow up appointments with the appriate teams if required.

As part of the Major Trauma East Lancashire NHS trust follow up process you will be offered a telephone follow up appointment with one of the Major Trauma Rehabilitation Coordinators.

Please note that this telephone appointment does not replace any other appointments that you have been given - please ensure that you still attend them all.

What to expect from your appointment?

On the day of your appointment you will be contacted by a call from an unknown number such as 01254 73200.  You will not be able to ring back on this number but please ensure that you are available to answer the call and be in a suitable quiet place for a conversation to take place.

During this telephone appointment, the Major Trauma Rehabilitation Coordinator will discuss with you your recovery, any current rehabilitation needs and ongoing rehabilitation goals.  

The Major Trauma Rehabilitation Coordinator will discuss with you any referrals that they feel may be required and action them if appropriate. 

The telephone call will last approximately 30 - 45 minutes.

Should you have any questions or concerns before your telephone appointment, please contact the Major Trauma Rehabilitation Coordinator Team who will be able to advise/sign post you to the most suitable service.
