Please contact the Major Trauma Rehabilitation Coordinator should you require any advice/support on discharge.  Please note this is not an emergency service and we aim to get to you within 48 hours of your query.

After discharge from hospital, you will be contacted by the Major Trauma Rehabilitation Coordinator Team to discuss your recovery and support any ongoing rehabilitation needs.

Should you have any questions or concerns before this, please contact the Major Trauma Rehabilitation Coordinator Team who will be able to advise/sign post you to the most suitable service.

What to expect as part of your hospital admission?

Royal Blackburn Hospital (RBH) is a Major Trauma Unit that is part of a wider major trauma network serving the surrounding areas.  Our Major Trauma service is designed to delivery high quality specialist treatment and rehabilitation to patients of all ages.  You may be transferred in or out of a local Major Trauma Centre (MTC) after initial treatment or start your journey here at RBH.  Our network MTC is Royal Preston Hospital.

During your stay you may meet our Major Trauma Rehabilitation Coordinator who can talk through any onward rehabilitation needs and support you and your family through these decisions.  They will work to ensure you are aware of your plan of care and what to expect throughout your hospital stay and on discharge.

it is common the first day of admission for the multi disciplinary team to start planning for your discharge.

Rehabilitation Prescription

You will be given a copy of your rehabilitation prescription whilst in hospital.  This document will be started on admission to hospital and updated during your stay.  It outlines your rehabilitation needs and goals to optimise your recovery if applicable. 

Discharge

Once you are both medically and therapy optimised for discharge from hospital, you will referred for all your follow up appointments with the appropriate teams.
