District Nurses provide nursing care for housebound patients at home. 

The District Nursing team comprises of a team of registered nurses, registered nurse associates, assistant practitioners, health care assistants and phlebotomists supported by administration colleagues. Each member of the team has an identity badge and will be happy for you to check this before you allow them entry into your home.

Your GP or hospital ward/clinic can refer you to the service. We also take referrals from adult social aervices and other health care professionals, as well as self-referrals from patients or carers.

The District Nursing Team provides care for people 18 years and over. The nurses will care for you in your own home or care home/day centre.

The service operates 24 hours per day, 365 days a year.

While we are unable to give specific appointment times, we will aim to be flexible where possible.

Your district nurse will review your clinical needs on a regular basis. They may recommend that your treatment and care be transferred to a more suitable service or, where appropriate, encourage you to manage your own treatment.

What to expect from the district nurse service

We will assess your individual health needs in partnership with yourself to identify the nursing care you require. This will involve you answering some health-related questions and agreeing a care plan.

We strive to ensure that:

  • Care is delivered with respect, privacy and dignity
  • Your nursing care will be based on your individual needs
  • You are encouraged to independently manage your health and care needs

Your personal information will be kept securely, however we may need to share some information with other professionals with your consent to help care for you

How can you help us:

  • If you need to cancel a visit please let the service know
  • We may ask you to remove pets when caring for you
  • Please do not smoke prior to or during visits
  • For infection control reasons nurses are required to wash their hands before and after care with patients. If possible please provide liquid soap and kitchen paper for the nurse to use
  • We will put our waste in a sealed bag which can be disposed of in your bin
  • We support health care professional training and your consent will be sought if a student nurse or doctor will be visiting you with us
  • Please let us know if you are unable to collect your prescriptions. While the service does not collect prescriptions, we can refer you to a collection and delivery service for pharmacy items
