The Community Rehabilitation Team works within the community to support patients that have deteriorated in their ability to manage at home because of a change in their health.
Referrals come from many sources. These include GPs, Consultants and the wider multi-disciplinary team such as District Nurses, Speech and Language Therapists and Integrated Neighborhood Teams.
We have locality bases to cover Blackburn with Darwen, Burnley, Hyndburn, Pendle, Ribble Valley and Rossendale.
The service provides person-centred assessment, advice and rehabilitation programmes involving the patient based on what matters to them about remaining independent within their own home and their local community. This includes those who have lost confidence and ability with their activities of daily living due to ill health or living with a long-term condition and wish to remain at home requiring some rehabilitation and therapy interventions to regain their skills and achieve their goals and maximum potential.
The service has a Head of Service and an operational lead and a clinical lead to support the service delivered is safe, effective and personal.
The service consists of the following staff:
Physiotherapists (PT)
A physiotherapist will help you to improve your movement, manage pain, and recover from injuries or illnesses affecting your physical function.
Occupational Therapists (OT)
Work with you to identify things that make it hard for you to do what you want to do in life. These could include physical difficulties or problems with memory and thinking, vision, or sensation, and problems such as anxiety. They will work with you to practice and develop your skills and confidence.
Assistant Practitioners and Therapy Assistants
Work alongside our Therapy Team to assist in the delIvery of services supporting clinicians, Occupational Therapists and Physiotherapists.

What we don’t do:
Assessment of adaptations (stairlifts, level access showers etc) and specialist equipment.
Referrals for equipment only.
Please note the Community Rehab Team is not an emergency or medical service – please ring the appropriate services if this is the case.
Opening times
The service operates from Monday to Friday 8am – 6pm.
